First Visit

During your first visit, you will meet with one of our experienced physical therapist for an initial evaluation.  The therapists will then explain your condition and your plan of care.  You can expect this visit to last 30 minutes to an hour.

We will have some paperwork for you to fill out and will need to take your insurance information.  Please arrive about 10-15 minutes early to your first appointment to allow time to complete the forms.  To speed up the process, you can also download the forms and bring them with you.

Prior to your first visit you will receive an email from Tx Xchange welcoming you to our clinic and explaining how to log in to your patient portal.  Please take a moment to familiarize yourself with Tx Xchange and feel free to message or call us with any questions.

Doctor’s Prescription (required only for Medicare)

Any Medical Records Your Therapist Needs to See (MRI’s, X-rays, etc.)

Comfortable Clothing

We are located at 2232 Pearl Street, right behind Pete's Electric Bikes on 23rd and Pearl. Entrance to the clinic is on the east side of the building 15 yards away from Pearl Street. The clinic can be accessed from the alley between Pearl and Walnut or from the walkway off of Pearl.
Free Parking is available along 23rd street or on Pearl Street.
