First Visit

During the first office visit, you will have a treatment by one of our highly qualified physical therapists. During this visit, your physical therapist will create a Treatment Plan that includes a combination of manual therapies and the therapeutic program that is right for you.

Before we get started, SOMA requires some forms and paperwork to be filled out so we can register you as a new patient. So please arrive 15 minutes early to help us with this process. We also need to make copies of your insurance card and physician prescription, so please remember to bring these items as well.

Comfortable Clothing

So that you feel comfortable learning postures and exercises at SOMA please bring comfortable clothing and shoes. Your physical therapist will help you plan to wear the proper clothing for your visits.

Your Planner or Calendar

To efficiently schedule your appointments, please bring your calendar to your first and subsequent appointments so your physical therapist can help you make the right appointments for your rehabilitation.
